About this support
Returning home after a hospital stay can take time and the right support. Transition care is designed to help you rebuild confidence and functional independence while you continue your recovery.
Care is tailored to your needs and goals and may include allied health, nursing, personal care and practical assistance for a short period.
The services available to you will depend on your aged care assessment, approved support plan, needs and budget.
How we can help
A recovery plan shaped around your goals

Nursing and personal care where approved
Help rebuilding strength and confidence
Coordination with your hospital discharge plan

Your recovery
Your transition care team coordinates and reviews the services in your plan, helping you work towards practical goals as your needs change during recovery.
Eligibility is decided through an aged care assessment, which is usually arranged while you are still in hospital.
